The Inpatient Social Worker carries a patient caseload at the 101 bed Psychiatric Hospital in Brooklyn Park, Minnesota, serving ages primarily 5-26. This role is part of a four year SAMHSA grant-funded position to primarily serve Native American patients. This role focuses on facilitating groups, treatment planning, discharge planning, care coordination, and communication with caretakers, community providers, and others involved in the care of patients. This role will specialize in easing Indigenous Health Disparities within the treatment of mental health and hospitalization, located within the hospital (75% of the time) and in the community (25% of the time). The role also includes community outreach to the Native American community and providing social work services across the continuum of care, including their return to the community.
