As a member of the Central Assessment Team, the Inpatient LADC/Patient Intake Coordinator, housed within the Brooklyn Park hospital, collaborates with the Central Intake Team, psychiatry providers and nursing staff to integrate relevant clinical data and substantiate medical necessity for recommended level of mental health care. This role is responsible for completing chemical dependency assessments and recommendations, as well as maintaining the referral list and follow through for current PrairieCare patients. This role is also responsible for coordinating admission to an inpatient level of care.
